Admission Procedures


DIRECT ADMISSION PROCEDURE

  • Patients with Doctor’s order for admission may proceed directly to Admitting Section of the hospital for immediate processing.
  • Upon completion of the Registration Forms, the Admitting Officer will provide information about the availability of rooms, hospital policies and procedures as well as the PhilHealth requirements for PhilHealth Members.
  • Ward Nurses are notified by the Admitting Officer of the admission. The patient and patient’s companion are asked to stay at the lobby while the room is being prepared. The E.R. Nurse will escort them into their room once it is ready.

ADMISSION THRU EMERGENCY ROOM

  • Medical emergencies, (unstable) patients and all pediatric cases must pass through Emergency Room for baseline assessment prior to admission.
  • After the need for admission is verified, patients or their authorized representative will be requested to accomplish two (2) sets of registration forms at the Admitting Section.
  • Upon submission of the registration forms, the Admitting Officer will provide information about the availability of rooms, hospital policies and procedures as well as the PhilHealth requirements for PhilHealth Menbers.
  • The Admitting Officer issues the patient identification bracelet that should be worn for the duration of the hospital stay.
  • Ward Nurses are notified by the Admitting Officer confirming the admission. The patient and patient’s companion are asked to stay at the lobby while the room is being prepared. The E.R. Nurse will escort them into their room once it is ready.

HMO ADMISSIONS


HMO Admissions either direct or thru Emergency Department are reported to HMO. Either the Admitting Officer or the ER Staff informs the concerned HMO Doctor Coordinator. Requirement for the HMO admission are :

  • Membership Card
  • Government Issued ID
  • Company ID, if Health Insurance is provided by the employer.
